The Wisconsin Department of Revenue has updated Forms for Retail and Temporary Alcohol Licensing. Forms were discontinued and new forms were issued. The Clerk’s office can only accept the WI DORs currently issued forms.

In 2024, Wisconsin law made significant changes to municipal licensing and regulation of cigarette, tobacco, and electronic vaping products. The Wisconsin Department of Revenue updated the forms that are required to be used in February 2026. The Clerk’s office can only accept the WI DORs currently issued forms.
